Follow-Up On Dental Screening

Dear Parents/Guardians,
In mid-November dental staff from the Community Health Center of SE Kansas
were here screening student’s teeth. At that time they applied fluoride to the
student’s teeth if the parents had consented.
In late January, 2012 they will return. They will clean and, if needed, apply sealant
to your child’s teeth. You will receive a report stating what services were provided to your child.
We will need your consent before your child’s teeth can be cleaned and the
sealant applied. We have the consent forms, a dental health form and a Medical History form, in the offices aat WBE, UJH and HS that can be picked up.

The free services will begin January 31, 2012.
