Sports Physicals for the 2026-27 School Year
Dear parents and guardians,
We will be offering FREE sports physicals to all current 6-11 grade students on Wednesday, May 6, 2026. If you would like your child to participate, there are forms available in the offices. Please return this form to the school no later than Monday, May 4, 2026.
*If your student is supposed to wear contact lenses or glasses, please ensure he/she wears them the day of the physicals.
